- An electronic Scope of Appointment form.
- Optional document uploads, when applicable.
- A final review and confirmation.
Recording notice
To help ensure accuracy, maintain required documentation, and comply with Medicare rules, Medicare-related calls and appointments may be recorded.
At the beginning of our appointment, I will let you know when the recording begins and ask for your verbal consent before we proceed.
Your recording will be securely stored as part of your confidential client file.
Before you begin
Please have the following available, when possible:
- Your Medicare card, if you already have one.
- Your current health insurance card or cards.
- A list of your prescription medications.
- The names of doctors or healthcare providers you want to keep.
Do not worry if you do not have everything available. You can still complete the packet and attend your appointment.
