Medicare by State

California has Medicare rules many other states don’t.

Medicare may be federal, but your state, county, ZIP code, doctors, prescriptions and timing can change what your real options look like.

The Medicare decision that worked perfectly for your cousin could be wrong for you.

This is where people get tripped up. Your cousin may love their Medicare plan, but they may live in another state, use different doctors, take different prescriptions or have different protections available to them.

In California, your options can be affected by state-specific rules, Medi-Cal coordination, county-based Medicare Advantage plans and whether you already have a Medicare Supplement.

California residents should know about:

  • The California Birthday Rule
  • Medi-Cal and Medicare coordination
  • County-based Medicare Advantage options
  • Medicare Supplement switching rules
  • What happens if you move or split time between states

California is not just “Medicare, but warmer.”

California has its own consumer protections, its own Medicaid program called Medi-Cal and local plan differences that can change from one county to the next.

California Medicare snapshot

This is the quick view. These are the California-specific issues I would want someone to understand before making Medicare decisions.

Topic What California residents should know
Medicaid program name California’s Medicaid program is called Medi-Cal.
Birthday Rule California has a Medicare Supplement Birthday Rule that may allow people who already have Medigap coverage to switch to another Medigap plan with equal or lesser benefits during a limited window without medical underwriting.
Medicare Advantage Medicare Advantage plans are local. Availability, networks, costs and benefits can vary by county and ZIP code.
Medi-Cal coordination Some Californians qualify for both Medicare and Medi-Cal. California also has Medi-Medi Plans in many counties for people who have both programs.
Moving or splitting time If you spend time outside California or are thinking about moving, provider networks, plan service areas and state protections may change.

Three California issues worth understanding.

These are not the only things that matter, but they are often where people realize Medicare is not one-size-fits-all.

The Birthday Rule

California gives many people who already have a Medicare Supplement a yearly birthday window to change to certain equal-or-lower benefit Medigap plans without medical underwriting.

Medi-Cal

Medi-Cal is California’s Medicaid program. If someone has both Medicare and Medi-Cal, the programs may coordinate differently than Medicare alone.

Local plans

Medicare Advantage plans are not one-size-fits-all across California. Your county, ZIP code, doctors, hospitals and prescriptions matter.

Thinking about leaving California part-time or full-time?

This matters if you live in California but spend time in Idaho, help a parent in another state or are thinking about moving to Florida, Texas or somewhere else.

Before you move or change coverage, understand how provider networks, Medicare Advantage service areas, Medigap rules and Medicaid programs may differ from state to state.

Questions to ask before choosing coverage

  • Are my doctors and hospitals in-network?
  • Do my prescriptions fit the plan?
  • Am I choosing Medicare Advantage or Original Medicare with a Supplement?
  • Do I qualify for Medi-Cal or help with Medicare costs?
  • Am I moving, traveling or splitting time between states?
