Why I created this
Families are often told to get the paperwork in order. The will. The DNR. The executor. The important documents. And yes, those things matter.
But when a loved one begins declining, the paperwork is only one piece of the story. Suddenly someone has to track medications, appointments, doctor questions, symptoms, supplies, contacts, services and the daily changes no one really prepared for.
This workbook was created for that in-between season, when your loved one may still be mentally sharp but physically declining, or when memory issues are just beginning and the family is trying to figure out what needs to happen next.
